Getting Access to the BMSC Questionnaire the first time

  1. If you are the PI, you should already have access to the BMSC questionnaire. However, if you find you are unable to submit it, send an email to ehsa@gatech.edu.
  2. If you are submitting a BMSC questionnaire on behalf of your PI and you're not able to access it, ask your PI to add you (following these instructions).
  3. If you are still having trouble accessing it, send an email to ehsa@gatech.edu, copying your PI, with the following information:

    View your BMSC Permits

    1. Go to the EHSA Website: https://ehsa.gatech.edu/ehsa. If the link for EHSA does not work, be sure you are connected to the GT or GTRI VPN.
    2. Log into the site using your GT credentials.
    3. Click on "Permits".
    4. Select the PI name from the drop down menu at the top of the page.
    5. All permits will show in the table below. 
    6. To only show BMSC permits, in the header row of the table, for "Protocol Type", type BMSC.

    Add CHEM users to permit

    This process will allow you to give EHSA access for new students or workers in your lab. This is ONLY applicable to chemical inventory and waste request modules, as BIO, LASER, RAM, and XRAY permits have additional requirements and a separate process to obtain access. Only Lab Managers and PIs have sufficient permissions to do this.

    Create Waste Profile

    In order to create a waste record, you must first have a valid waste request profile. You will need to edit your waste profile to include additional PIs if you don't see them in the drop-down (after clicked "Edit" next to "PI" in the top part of the waste request screen.

    Navigating the BMSC Application Form

    • There are two sections that must be complete in order to submit a protocol.  Each section can be accessed via the tabs on the left side of the screen, titled "Permit Process Questions" and "DURC & Acknowledgement".
    • All questions must be completed to submit the application.  Questions that are incomplete upon moving to another section will be highlighted in red on return, and the number of open questions will be listed under its respective tab on the left.
