Read the incident below, and see if you can determine how it could have been avoided:
A lab worker was disposing of acid waste into a plastic bottle labeled “acid waste”. He was wearing appropriate PPE. When the bottle was full, he placed it into the cabinet under the chemical fume hood. He went back into the lab around noon to pick up his lab notebook, not wearing any PPE. He heard a noise coming from the cabinet under the chemical fume hood. When he opened the cabinet door, the lid popped off the plastic bottle, splashing a small amount of the waste onto his face. He immediately attempted to rinse his eyes using the sink eye wash. He was able to rinse his eyes for 2-4 minutes, even though there were items blocking the eye wash in the sink. Later, after the eye became red and irritated, he went to the doctor and was treated. Luckily, no damage to his eyes occurred.
What went wrong? We came up with 5 things that either led to this eye exposure or could have been done better. How many can you come up with?
