Consumption Tracking Form (daily)

Urgent Request Form

Lab Inventory (one time)

What we ask you to do now
  • Log your daily consumption of gloves and surgical masks using the consumption tracking form.
  • Log when you finish a bottle of hand sanitizer or disinfectant using the consumption tracking form.
  • Submit an urgent request (if you are about to run out of gloves or surgical masks, or if you have finished a bottle of hand sanitizer or disinfectant and don't have a replacement) using the urgent request form. For the short term, this form will also be used for labs to indicate what they need if they haven't started tracking their consumption yet.
  • Have one person in the lab submit the lab inventory form so that we know what you have in the lab and what you need.
Overall process flow

Lab users will record their daily usage of gloves (number of pairs), hand sanitizer (when they empty their 4 oz. bottle), and disinfectant (if there is a spray bottle in the lab) using the consumption tracking form. This information will be consolidated for each room/location, and a list of replenishment orders will be generated. These supplies, from the building supply, will be placed in the lab frequently so as to avoid most shortages.

As items go from the building supply to the labs, this will generate building replenishment orders that will be filled from a central supply (currently EHS) in a similar manner. This eliminates PIs estimating what they will need, getting all of that information to a building manager, the building manager summarizing it and requesting it from EHS, EHS pulling and scheduling the pickups, and the building managers picking up supplies from EHS.

In the case of a larger use of supplies than anticipated, an urgent request can be made using the urgent request form. The SCL/PI Center group and EHS are monitoring the requests and will notify the building manager (or point of contact), who will get the items to the lab promptly.
